Excellent question. In that case you have different possibilities.
a) Customer A's SMSC never know if Customer B is online and simply
retries every 15 minutes. This often happens if you are roaming
b) Customer A's SMSC has access to Customer B's home location
register (HLR) and it will inform Customer A's SMSC when Customer B
gets online.

The flag doesnt help if messages are waiting. The could be waiting on
10 different SMSC's. The HLR would have to keep track which SMSC has
messages waiting for the customer. I think this only really works for
your home SMSC but not for roaming SMSC.

Hes happened to me once and I couldnt reply because the SMSC in use
(PacBell) was blocking me from going back to my friends phone over
them. The options in the phone say something like "Sender pays for
reply" meaning the sender adds an information that replies should go
over his home SMSC and he pays for it. That implies the recipient to
use A's SMSC for the path back. Happened to me but didnt work
presumably because the SMSC wasnt configured. for reverse billing.
