merged from private version into official.

Please test attached patch against CVS head or checkout/follow from/on:
        http://github.com/amalysh/kannel/tree/smpp-throttling


This patch implements following:
        - new main loop logic
        - some inlines for speed
        - more error handling
        - async shutdown

Awaiting comments ;)
