We use o11s-dashboard.  See an example here:

You will see it's very crude yet but the source is available here:

I know little about Nagios but apparently it connects to the monitored
machines via ssl or ssh
(http://nagios.sourceforge.net/docs/3_0/addons.html#nrpe).  If your
monitoring host keeps connections to all the nodes in your mesh
network, it is possible to monitor the mesh topology in real time,
yes.
