Due bogus usage of signal handler, if the startup fails in dont_fork mode, the
cleanup routine will be triggered multiple times and deadlocks will most
probably occur.
This bug affects all versions, has a higher rate in 2.1 (due the new
architecture) and it was spotted while fixing #445 .
