Did we log the KOD packet?

What are you doing to trigger a rate_exceeded?  Is there another bug here?

The default is 8 seconds. You are sending every 16 seconds.  Are you also 
running extra requests?  For example from ntpdig/ntpdate?

ntpq and ntpmon show hpoll which is the nominal polling interval, but there 
are some additional considerations.
