While looking into a bug in RHEL6 (which turned out to be fixed by
upstream commit 112f661d6dac9af1235d2d05299fc2c9cb876ae7), we ran into a few
other minor bugs (lack of locking and triggered WARN_ON's). This patch series
takes care of those issues
