- changes in usr_preferences keys/indexes:
1) primary keys extended to
(uuid,username,domain,attribute,type,value)
to allow AVPs with multiple values
2) indexes on (uuid,attribute) and ($USERCOL,domain,attribute) to
speed up
the loading
