I had this in mind, but as there is no need for it in 1.3 (no module to
use it), I though it make sense to let it for 1.4 when we can have
modules actually using it.
And for the MI functions - they can be called only from outside.

- added new module parameter "dlg_extra_hdrs" for adding custom headers for
the BYE requests generated by the module
- the MI "terminate_dialog" take a third optiona parameter "extra_hdrs" to
add various extra hdrs in BYEs
- some clenaup and extra checks
- fixed some bugs related to mem handling
