It seems OK to me, the problem I see with it at a quick glance is this clause:

Not mimic the style and appearance of the original information, for
example by replicating the look and feel of a published document or a
departmental website.
Which as far as I can tell would stop, say, WhatDoTheyKnow reproducing
a document that was an FOI response verbatim if licensed using the
license.
I would add that "Not re-use the information for promotional or
advertising purposes;" is likely to cause trouble as well, given
there is anyway "Not re-use the information in a deliberately
misleading way" which covers most times that would matter.
