I have 
faced this problem in 2.5 but not in 3.0. This was happening at random and 
just once in a while so we could not investigate this 
matter.
 
What 
version are u using?

  deposits somehow became an uncleared transaction.  Now the bank 
  reconciled balance is incorrect.  If you go back to the 
  reconciliation where the item was cleared it says that the 
  reconciliation was off exactly that amount.How can I correct
