I have the same problem with the transactions. I think the problem is with the 
transaction that made the server (3 tier), is too much for the existing 
hardware (for this reason Microsoft can�t reporduce the error), and lost the 
sequence, or the timeout happend.
 
The only solution that I do was increase the hardware, and not happend again. 
Remember that in 3 tier your client is dummy, and all work is doing by your 
server.
