Have you checked the Number Sequence, and can you let us know in which module/menu option and which number sequence like Voucher Number sequence already used .....". Voucher number is most of the time a number sequence which implementers don't take care about and get problems in Live Implementation sites.
My suggestion will be to check which Number Sequence is a problem , which you can find out from the Infolog, and have a look at the Sequence and may be your option will be to put a prefix to it.
