Hi Stephan,
Have you checked out the AxaptaError.log file yet,
sometimes specific crash log info is recorded there.if that doesn't help you
could possibly use any of this 3 startup (ax32.exe -parameters) modes :
-noauto :
ability to bypass startup code, and
some timer based calls.
For example, if code is introduced in the startup method that causes Microsoft
Axapta to go into infinite loop, and therefore, never finishes the startup
procedure. To change this, start with the −NOAUTO switch, correct the code,
and restart without the −NOAUTO to have the startup code included again.
-retry<n> (n = no.of seconds)
Delay in seconds before re-executing after a deadlock.
-repair
Use this option to handle situations when problems in SQL system tables
prevent Microsoft Axapta from starting, for example missing indexes. It enforces
synchronization.
Try these and best of luck!

Subject: [development-axapta] Axapta crashes after table change, can't open
afterwards
Good day,
We have an Ax3.0 SP5 KR2 installation – after I have made changes
(added a field) to the VendTable, Axapta crashed with no error message
and no information in the Event viewer related to the crash. Now, when
I try to open Axapta, I am presented with the logon form but it
crashes immediately after logon – even before the main menu loads.
Again no error message.
I have tried a couple of things with no success:
1. Restored the application and database (to before change on
VendTable) from a backup, recompiled, synchronized the db – made the
change again and Axapta crashes again
2. Restored again, removed the axapd.aoi to force Axapta to rebuild
the index, recompiled – made the change again and Axapta crashes….
3. Restored again, started a re-index, but the system crashes again,
even before I make the VendTable change
We have changes on the BUS, BUP, CUS and USR layer.
Can I force Axapta to refresh its indexes at startup to sort this out?
What parameters are available for startup (ax32.exe -parameter) to
maybe force Axapta to start in Configuration mode? Or something like
that… How can I trace what causes this error?
Thanks,
Stephan
