Another thing to look out for:
if you want to restrict access by NT group, make sure
the proxyauth file is located on an NTFS partition. 
On a few of my servers, my \netlogon share is on a FAT
partition and you can't assign NT permissions on a FAT
partition.  I created a separate share called LINUX on
an NTFS paritition and assigned read rights to the
file for the NT group that I wanted to allow out
through squid.

I also compiled smb_auth myself instead of using the
rpm so I can't comment about that.
