Any time TreeRoutingTable.decreaseContactProbability is called counts as a failure. Some causes are connection failure / timeout (i.e. can get an outbound connection at all). Authentication errors, meaning the link crypto didn't work. And restart meaning the connection was established but the target node failed to respond to a message within the timeout interval. I think that there might be a bug which causes links to go bad over time.
