Oskar:
I don't think this is as bad as it looks.

There was a bug that caused connection handlers to get taken out of the OCM's 
table without being taken out of the LRU queue.

I fixed it and also made CH.terminate() unconditionally remove the connection 
handler from the OCM as a defensive coding measure.
