Hi. The output from wait-for-couch-db suggests that something went wrong with the init-couchdb-job and the tests_activations was not created. (The expected output from wait-for-couchdb is to print that wget error message some number of time (20 to 30 is not unusual), then eventually successfully do a wget on the READINESS_URL and exit (indicating that the database is now ready).
Could you get the logs from the couchdb pod and the init-couch-db job to see if they have any hints? In parallel, I will also improve the output from the wait-for-couchdb container to perhaps make it easier to recognize the expected output.
