You may want to leave some space for "recently often used functions" in such a toolbar where almost everything is too level away?
Also, some of the inserts dialogs could, in an ideal world, be "put aside for a while" (e.g. on the right) until they are used? (e.g. I am about to insert a picture, and have everything ready for it, but I need to insert a frame and caption before).
