...to be able to give you a better explanation of what is happening, I'll
need to see more of your dhcpd.conf file:  the host entry is correct, but I
have no idea if you have correctly declared subnets and shared-networks.

...I'm also not familiar with the messages generated by Windows NT for the
DHCP server, but it sounds as if the Windows client did not receive a
response to a message (likely either DHCPDISCOVER or DHCPREQUEST), which is
why I asked about the dhcpd.conf file.  Could you also show us the relevant
part of syslog and other data (like the output of tcpdump or a sniffer)?  If
there are no syslog messages showing the receipt of a packet from this
client, the problem lies external to the DHCP server, and a packet trace
will help find the trouble.

assigned (or offered) by the server.  Think about it:  fixed-address
assignments are time-invariant, so there is no information necessary apart
from their appearance in the dhcpd.conf file.  The dhcpd.leases file is a
transaction log of dynamic address allocations managed by the server.
