Hi, Jason. I am trying to figure out this issue. I have downloaded Adobe Reader 11.0 which is the current version available for download. With the default setting, I was able to fill out the PDF form in the Adobe Reader.
Could you download Adobe reader version 11.0 (http://get.adobe.com/reader/) and try filling out the PDF form?
