api dataValueSet error NullPointerException

Bug description:
The api resource dataValueSet returns an error when the parameter
dataElementIdScheme=code is passed and a data element doesn't have a
code.
To replicate this behavior in http://apps.dhis2.org/demo delete the
code from the data element "OPV0 doses given" of the data set "Child
Health" (BfMAe6Itzgt) and open the api resource with the following
link:
http://apps.dhis2.org/demo/api/dataSets/BfMAe6Itzgt/dataValueSet?dataElementIdScheme=code
Tested in DHIS v2.15 build 15033
