just to ask, are you using the custom data entry form or default? and how many 
fields do appear on the form when you are doing data entry?

my point is if you are using the custom data entry forms, and the number of 
fields appearing on data entry are six (in case the report underneath means 
actual is all the the fields ) then that might mean there are some data element 
assigned to that dataset and have not been assign to data entry form. though 
this will be completely different if you are using default form.
