Thank you Georgi for your response. So I guess the answer is that you cannot reference a specific dataelement+attributeoptioncombo when setting up an indicator in order to calculate a very specific rate. Disaggregation in the report will in this case not work as far as I can see because the denominator is not disaggregated by that attributeoptioncombo. I just wanted to make very sure that was not missing a feature in DHIS2 which I was not aware of.
