Is there a Windows programming styleguide for this issue?

Hm, apparently there is, it's called "The Windows Interface
Guidelines for Software Design: An Application Design Guide", and you
can buy it in a book store. :-\

Microsoft's online guide "User Interface Design and Development"
us/dnanchor/html/anch_uidesigndev.asp>) contains stuff about menus,
but a quick glance did not reveal any usability considerations. I did
not look very hard, though.
