The usual groups who are part of the digital divide face this issue - clinics that serve low income patients and rural clinics, as well as the small offices that Andy mentioned. The Indian Health Service has come up with a model program http://www.ihs.gov/CIO/EHR/ I don't know anything about this program, except that it exists.
