The kernel soundmodem driver that you described is now deprecated. There 
is the "userland" soundmodem driver that runs as a program:

You still have to set up the axports file as you described, but the 
soundmodemconfig program sets up the other settings (callsign, txdelay, 
etc.).  In the end, you just run soundmodem, and it creates the sm0 
interface and attaches it to the matching entry in your axports file.

It works with any sound card that works in Linux, with either OSS-style 
programming or the newer ALSA-style sound system interface. You can even 
run multiple modems on the same channel, etc.
