Cleaned up the way CreateSurface() determines the primary surface
pixelformat and size. Previously the behavior varied depending on
cooperative level and the force-desktop option.
New behavior:
1. DSDESC_{WIDTH,HEIGTH,PIXELFORMAT}
2. SetVideoMode()
3. mode= and pixelformat= options
4. current layer config
