This PR adds `policy = expression` support to `#[dispatch(visit)]` and `#[dispatch(mutate)]`. Policies share the dispatch object's state and run when no handler matches or a handler requests default recursion. Children still go through normal callback dispatch. It also preserves an outer `Pattern` during recursive visits and avoids duplicate error frames without stopping at existing null entries.
