I have tried three times to transfer a domain from Domain Bank, and each
time the request times out waiting for the losing registrar to respond.
However, the NSI-Registrar License and Agreement posted on ICANN's
website states:

"In all cases, the losing Registrar shall respond to the e-mail notice
regarding the "transfer" request within five (5) days. Failure to
respond will result in a default "approval" of the "transfer."

So my question is, why is the transfer timing out and failing, instead
of defaulting to a successful transfer after 5 days according to the
agreement? I have verified that the domain in question is over 60 days
old and is fully paid up and not "on Hold" for any other reason.

Anyone have any ideas?
