You don't use the host form at NSI. That is only used if the parent
domain is registered with NSI as well.
You register your nameserver host in the manage domains interface.
Click on Nameservers, and then at the bottom of that page is the link
to create/edit a host based on this domain. Click on it and follow
the instructions.
NSI's Registrar will let you use it once it has been created in the
Registry.
