I've been noticing the same tonight. I was resubmitting applications
that failed during this afternoon's outage.
I would get a "domain taken" error, and then reload the page and it
would go through (we use our own hooks into OpenSRS.pm, I'm talking
about our own thing which was a pre-cursor to Colin's php version)
It would happen like this about 70% of the time. I was suspecting the
problem was on the opensrs side.
