Tucows generates random auth codes for each EPP-based domain -- .info, 
.biz and soon .name and .us.  It's a good guard against hijacking.

Each registrar has their own system of generating auth codes.  It 
wouldn't surprise me if some lesser tech-savvy registrars used (or used 
to use) the same auth code for every domain (or maybe something 
predictable, like the date maybe).  But I would imagine that most of the 
ones which are playing unfair have random generators as well.
