It depends on the domain and nameserver you're trying to do.  Consider the
following scenarios:

1) domain exists in .com registry, nameserver does not.  You must use the
"Registry Nameservers" link in the RWI to "populate" the .com registry with
the nameserver before you use it... this works for all combinations (i.e.
.info domain, non-.info nameserver; .cc domain, non-.cc nameserver, etc)

2) domain exists in the .com registry, nameserver exists in the .com
registry.  You must use Manage (https://manage.opensrs.net/) and under the
"Nameserver" section is a link (at the bototm of the page) to
"create/modify" a nameserver based on your domain.  This will create the
necessary host.  This applies to other combinations (i.e. .biz domain, .biz
based nameserver; .net domain, .net based nameserver, etc)
