There are systems that can make use of shared block devices. For example,
in Linux there is ocfs2 and gfs2 which use a shared block device for a file
system. Unlike regular file systems, the clients have to play nice using a
distributed lock manager to not corrupt the volume. One could teach any
block device accessing sw (fs, db, etc.) to play nice with others with iSCSI
disks, but it's a fair amount of effort and people generally don't care.
