I suspect the root issue was never resolved. You should review your memory allocation; also, older mpt adapters may have a requirement to use memory for DMA with certain restrictions. This memory can become scarce, even when there is plenty of main memory still available. It gets worse as you add more adapters into the system.
If you can update to an mpt_sas based adapter, you'll probably have better luck. I've not checked the code, but my guess is that on a 64-bit kernel that hardware can probably address the full 64-bit memory space, meaning you won't see the problem unless your main memory is horribly, terribly fragmented (unlikely) or you've exhausted all the main system memory (a real situation which can occur, but which is often solved by adding more RAM, or adjusting your workload to reduce memory pressure.)
