This survey really isn't very good unless you work within a corporation and perform usability testing.
I have my own company and don't do usability tests, so I had to leave quite a few answers about testing processes blank. I had to specify that I was both an Executive and a Consultant, hoping that someone can extrapolate from there. I also specified my daily rate, but the survey didn't ask how many days I bill out in a year. How can someone gauge my salary without a way to determine my annual income? Eh. Just another survey that is less meaningful than it should be.
