It depends :-)

Actually, when I can, I prefer to put nothing in the usability report.

Instead I use the time that would have spent writing things up with  
the rest of the development team - explaining and demonstrating  
problems and issues directly.

Much more effective than a lump of paper in my experience (and often  
quicker!)
