One thing I've noticed (after several trial-and-error attempts) is
that you don't want the f2f interview to be a re-run of the phone
interview. So, what I've been doing in the latest interviews is
pretty much ask most of the questions over the phone and leave the
face-to-face for interactions I couldn't do over the phone, like
asking the candidate to solve simple interaction design problems,
whiteboard samples of workflows, etc.
That changes the dynamics of the face-to-face a bit: since most of
the questions have been asked already, I take my time during the f2f
to look for the candidate's attitude: his behavior during meetings
(usually our f2f involve several people), the way of handling
critiques, motivation, etc.
