Hi,

A small procedural point here, if you ask people to add information
during group brainstorming that will reduce the number of ideas and
distract people.  Since the goal of brainstorming is sheer volume (and
the research generally indicates that sheer volume is correlated with
quality), you might want to consider brainstorming first,then adding a
stick-on template for additional information like positive and
negative issues associated with the idea or rankings/priorities.
Another thought about putting ideas on paper is to use the magnetic
paper that you can buy at Staples or other office supply stores.

I do worry about anything that distracts people from brainstorming,
even labels on templates that might get people to think about the idea
rather than generating new ideas.

There are some forms for individual brainstorming plus other comments
-- The Crawford Slip Method is a bit unusual, but can be effective for
large group and has a template.  There are some forms for individual
brainwriting that are useful.  I'll see if I can dig those up.
