Hello Jared, there are tons of examples on a UCD approach that went wrong. A successful UCD approach is where the cross functional team does a heavy amount of "international" UX research, IA activity and constant usability tests throughout the development process NOT neglecting proper marketing. For a product to be sucsessful proper marketing is needed. Many businesses forget this. Also many teams do not conduct a heavy amount of research before development and jump into designing after a few qualitative research activities. The management is too eager and cannot wait to launch the product, due to deciding a lauch date way to early. Patience is what is needed. I know that I "might need a reality check" but from what I've experienced at work this is what I see. (part1)
