You can have a look at (www.usabilitylabrental.com), Noldus Inc
(www.noldus.com) and Ovo Studios (www.ovostudios.com). BioObserve
(www.usability.biobserve.com) do seem to have a good usability lab
setup, but they are quite pricey. I have not worked on setting up a
fixed lab but i have worked on researching about mobile labs. These
three firms offer pretty good services and they do have solutions in
fixed lab set up as well. 

Morae (www.techsmith.com) is a industry standard software for
usability testing.

Other offering you might be interested in is Tobii eye tracking
(www.tobii.com) which is really fancy stuff.

You will have to watch out for over spec-ing your lab and need to
tailor your lab according to your requirements.
