I have some books, but nothing I remember sufficiently to recommend. However, I do recommend the writing of Brad Appleton (et al.) on branching patterns: http://www.cmcrossroads.com/bradapp/acme/branching/ It appears that they have written a book of patterns including some information on release management, but I can't speak to that. (I seem to be missing all of your points; that set of articles is over ten years old.
