I've created a callout template and view based on the above checklist. You
can check it out here:

I've use the image_mini version of the uploaded image, since using a wider
image could create problems with rather big urls (overlapping). It is an
automatically created 200px wide thumbnail of the uploaded image. We can
also use image_preview which is a 400px wide auto-generated thumbnail, or
use the original image uploaded. But I feel that 400px is too wide and by
using the original image, we can not control the width of the uploaded
images.
