#692: List Countries More Logically - Listings and Video Publish Form

The list of countries looks a bit messy. Perhaps at one point it was
listed in order of region, but there is no clue to this in the listing
currently, and things aren't in order.
If we are going to list these by region, it would be good to have
separators in both the video publish form, and the countries listing, that
are headings for each region.
Note: Plumi currently seems to use a similar list of countries as
EngageMedia - should we make Plumi countries all countries, by default,
and only customise the list for EngageMedia?
