I don't mind the agenda being made up on the spot, as long as its still dealt with one item at a time -- if a new item comes up mid-item it can be tacked on the agenda for later and the current item brought to completion first.

What I'd like to see is organized turn taking for speaking, either the meeting chair or an assistant should keep an eye out for whoever wants to jump in, give them a visual ack that they're in the queue, announce who's next when its time, and sometimes even announce where the queue is at so that people can appreciate there's a queue.
