I have noticed similar behavior. I think that the playlists are not updated until the end of a rescan. (This is a guess, I haven't looked at the code...)
Rescanning a large library takes a long time, which makes minor updates in iTunes (tag edits, playlist adjustments, etc.) painful since the slimserver has to rescan everything.
