I've saved internet radio stations in playlists for easy access and this
works fine in all cases except Live365.

I'm a VIP user for ad-free listening and I presume that saving a radio
station in a playlist simply saves the current URL. In the case of a
VIP login to a Live365 station, this URL has your unique login session
info embedded in it, and if you just keep using it, after a while (a
few days) the session ID times out and you get a recorded message
telling you so.

This is how the saved playlist behaves, so I assume this is why.

I can't imagine that there is any sensible way around this. Instead of
saving the current URL, you would have to send some kind of string with
actual login information in it, ie what the Internet Radio plugin does
now when you access a station and you have your login and password
saved on the server.

This means that basically the quickest way of listening to Live365
stations with a VIP account is to have them in your favorites list at
Live365 and access them via the Internet Radio plugin interface. A few
more button-presses, but it works!
