Note that port-forwarding to devices works differently on different routers. My UK Netgear DG834 won't forward to devices that aren't 'attached' to the device - ie, not powered on. So I couldn't wake mine from a device outside of my home LAN.
Regarding MAC addresses, I'd think most routers would list them alongside IP addresses. Certainly, the Netgear does (under attached devices). If you don't have a router, IPCONFIG /ALL will tell you - you just need to identify the correct IP address and take the MAC that goes with that.
