Curiouser and curiouser... I would turn on playlist debugging, create a fresh playlist in SC, save it, then do a playlist only rescan. Then have a look at scanner.log. It should give you an idea where things are going wrong.
I would still guess at permissions - because the type of behaviour you describe would normally occur where SC scans the file but can't access it so removes it from the library, but what you set out below looks right to me.
