I haven't seen anyone reply to this part of the reported problem. I routinely add tracks to playlists, and then save the playlist with the same name. It prompts me to make sure I really want to overwrite the previous playlist, but if I confirm that I do, it saves it with no problem.
This is via the web interface -- I haven't tried it via remote control. As other have stated, I far prefer to use the computer for playlist creation.
